PANUKALA NG MGA TAGA CULION



PAKIUSAP SA MAKATWIRANG PANUKALA NG MGA TAGA CULION

Hon. Emiliano P. Marasigan, Mayor ng Culion at sa aming Honorables Sangguniang Bayan Members at sa Lahat ng Kinauukulan:

Kami pong mga mamamayan ng Culion ay nakikidalamhati sa bigla at maagang pagkawala ng ating minamahal na DR. PAUL A. EVANGELISTA. Simula pa po ng kanyang kabataan, kinakitaan na po siya ng matinding pagmamahal at pagmamalasakit sa mamamayan ng Culion. Ang kanyang pagiging mabait na manggagamot at masipag na siruhano, noong siya ay nabubuhay pa, na nakapagbigay kaligayahan sa mga maysakit natin kababayan ay talaga pong hindi matatawaran. Dahil po dito, siya rin ay nakilala sa Ibat ibang karatig munisipyo ng Culion.

Nais po naming manatili ang mga magagandang alaala at mataas na paggalang kay DR. PAUL A. EVANGELISTA, sa aming mga puso at isipan. Nais din po namin na manatili Ito sa aming mga anak, mga apo at sa mga susunod pang henerasyon.

Dahil po dito, kaming mga mamamayan ng Culion ay buong pagpapakumbaba at nakikiusap na palitan ang kasalukuyang pangalan ng Culion Sanitarium and General Hospital sa DR. PAUL A. EVANGELISTA MEMORIAL AND GENERAL HOSPITAL, bilang paggalang, pagpapasalamat sa kanyang mga nagawang kabutihan sa mamamayan ng Culion. Hinding hindi na po natin siya malilimutan.

Maraming salamat po sa inyong agarang pagkilos tungkol dito.

A petition addressed to the Honorable members of the Sanguniang Bayan of Culion to pass a resolution requesting the Department of Health to declare and rename the Culion Sanitarium General Hospital as Dr. Paul A. Evangelista Memorial General Hospital

PETITION
We, the undersigned in recognition of the great deeds, sacrifices and dedication of the late DR. PAUL A. EVANGELISTA as a public servant, physician and kababayan do hereby manifest to petition the Department of Health thru our Local Government of Culion the conversion of the name, Culion Sanitarium and General Hospital to Dr. PAUL  A. EVANGELISTA Memorial and General Hospital…

Whereas,.. the late Dr. Paul Evangelista epitomizes what Culion was all about… a place to serve and dedicate.

Whereas, as a physician he made a lifelong sacrifice to give not only care and devotion to his profession but a firm believe that Culion will live up to its commitment to us as a people and as a place we can all be proud of

Whereas; as a kababayan, where the past is our foundation he help build as a community of people who care not only our constituent but to the others in need and caring; instilling in us the same commitment, that to live is to serve and to serve is to love

Whereas; Dr. Paul Evangelista as a true measure of a man,  unite us as sons and daughter of Culion to be dedicated to the great task remaining before us sharing that dream and hope that we can still achieve a better tomorrow if we just hold on together

Whereas; Dr. Paul help facilitate in the conversion of Culion  Sanitarium to a Municipality and the conversion of the hospital into Culion Sanitarium and General Hospital ,
Whereas; Dr. PAE continued to serve Culion even after retirement as a physician doing volunteer work to the hospital

Whereas; as a true public servant as Vice Mayor he never ever wavered to give free consultations and medical mission until his dying moment

Whereas; we are indebted to this greatness

Now therefore, be it,

Resolved, as we highly resolved to PETITION the Department of Health thru the Local Government of Culion the Conversion of the title Culion Sanitarium and General Hospital into DR. PAUL  A. EVANGELISTA MEMORIAL HOSPITAL as our gesture of recognition to the great deeds and sacrifice of this man to the general welfare of the people as a doctor and a public servant

Resolved further; to request the Sanguniang Bayan of Culion to pass a resolution adopting this petition as representative of the people

Resolved finally to furnish all concern department, agencies and offices for information, reference and appropriate action
